Oakland, N.J., Jan 6 2022 \u202f<\/strong>\u2014<\/strong>\u202f<\/span>Building on its commitment to\u202fcomprehensive\u202fcare and services,\u202fOakland Rehabilitation & Healthcare Center<\/a>\u202fin Bergen County recently earned\u202fthe American Heart Association\u2019s (AHA\u2019s) new Skilled Nursing Facility Heart Failure Center Certification<\/a>.\u202fLocated at 20 Breakneck Road in Oakland, the 215-bed\u202fskilled nursing facility\u202fprovides post-hospital care, short-term\u202frehab\u202fand long-term residential care.<\/p>\n

The world\u2019s leading nonprofit focused on heart and brain health, the\u202fAHA<\/a>\u202fhas invested more than\u202f$4.8 billion in research since its founding.\u202fEvaluation criteria for its new skilled nursing certification \u2013 earned by less than 1% of skilled nursing facilities nationwide, to date \u2013 was designed by heart failure experts. Certification requirements include program management, personnel education, clinical management, care coordination and performance improvement, as well as patient and caregiver education and support.<\/p>\n

\u201cThis advanced certification\u202fprovides heart patients with the assurance that Oakland is\u202frecognized by the\u202fAHA\u202ffor delivering the most effective heart failure treatment strategies\u202fbased on a standardized method of current evidence-based guidelines,\u201d\u202fsaid Ugochi Opara, regional director of market development at Marquis Health Consulting Services, which supports the facility.\u202f\u201cThese specific care strategies\u202fand services\u202fare aimed at improving outcomes for those with chronic heart failure and its co-morbidities, as well as reducing readmissions.\u201d<\/p>\n
